NOTE: Incomplete forms or forms without supporting documentation will not be processed. Supporting documentation required is listed on the form.


To expedite the approval process for your entitlement form, we recommend that you use our upload functionality to provide the completed form and any supporting documentation. To upload the form and other documents, simply sign in to your ECSI user profile, access your loan account(s), go to the Documents tab and click on the Upload button. Please note that you may only use the upload functionality if the form is being returned to ECSI. If the form is being returned to your school, you will need to mail or fax the form according to the instructions on the form.

Once you have downloaded and completed the entitlement form, you can come back to upload the completed form and supporting documentation. Please note that we only accept uploaded entitlement documents if they are processed or mailed to ECSI. If your form includes a Mail To address of your school, you must mail the document to the address on the form.
